00:00:33: In today’s lesson, we will cover the most common restorative and pulp treatments in pediatric dentistry. So, let’s start! The aim of restorative treatments in children is to preserve tooth structure wherever possible, restore function, and prevent the spread of infection — at the same time keeping the child comfortable and building a positive relationship with dental care. Ziel restaurativer Behandlungen bei Kindern ist es, so viel Zahnsubstanz wie möglich zu erhalten, die Funktion der Zähne wiederherzustellen und die Ausbreitung von Infektionen zu verhindern. Gleichzeitig geht es darum, dass sich das Kind während der Behandlung wohlfühlt und eine positive Beziehung zur zahnmedizinischen Versorgung entwickelt.

00:01:36: For primary teeth, the treatment choice depends on the extent of the caries, the child's age and cooperation level, and how much longer the tooth is expected to remain in the mouth before second dentition. A tooth that will fall out in six months may be managed differently than one that will remain for another four years. The dentist can say: „We always try to save the baby tooth for as long as possible, because it's holding the space for the adult tooth that will come through later.“

00:02:24: Number 1 - Composite Fillings in Primary Teeth: For small to moderate cavities in primary teeth, tooth-colored composite resin fillings are the material of choice. How to explain it to the parents: "Your child has a small cavity in one of the back baby teeth. We're going to clean out the decay and fill the hole with a white filling material, so the tooth is nice and strong again. We'll use some sleepy juice first to make sure the tooth is completely numb, so they won't feel a thing."

00:03:16: Children-friendly phrases: "One of your teeth has a little sugar bug hole in it — and today we're going to clean it out and fix it up so it's good as new!" "We're going to give your tooth a little repair job today, just like fixing a hole in your favorite toy!" "I'm going to clean out the poorly part of your tooth and fill it with a special white material that will make it strong and healthy again." "First, I'm going to put some magic sleepy cream on your gum — it tastes a bit like strawberry, and it makes everything go numb, so you won't feel a thing!" "Now I'm going to give your tooth a little sleep help, so it has a nice long nap while we fix it. You might feel a tiny bit of pressure, like someone pressing on your gum, but no sharp pain — okay?" "Can you feel that? No? Perfect — your tooth is fast asleep and ready for its repair!" "I'm going to use my little cleaning tool now to remove the sugar bug hole — you might hear a buzzing sound and feel a little vibration, but your tooth is sleeping so it won't hurt!"

00:05:04: "Now I'm washing your tooth with a little shower of water — Mr. Thirsty is going to drink it all up for you!" "I'm painting on a special sticky liquid now to help the filling stick really well — just like glue for your tooth!" "Now comes the filling — I'm going to put in the special white material and shape it to look just like your real tooth. Nobody will even be able to tell it's there!" "And now my magic blue light is going to make the filling so super hard in just a few seconds — it's completely safe and won't hurt at all!" "Can you bite down on this special paper for me? It's going to show me exactly where your teeth touch so I can make sure your filling feels just right." "Does it feel like something is in the way when you bite? Great — then everything is perfect!" "Your mouth might feel a little numb and funny for the next hour or so — that's completely normal, it just means the sleepy juice is still working. Try not to bite your lip or cheek while it's still numb!" "You were absolutely brilliant today — I'm so proud of how brave you were!"

00:07:20: Number 2 – Pulp Therapy: When caries reaches the nerve, pulp therapy is required. In primary teeth, there are two main treatment options: pulpotomy and pulpectomy. Parent-friendly phrases: "I know it sounds worrying when I mention the nerve of the tooth, but this is a very routine procedure in children. It is not the same as a root canal in an adult tooth — it's much simpler and quicker, and most children tolerate it very well." "The goal is always to keep the tooth in place until it's ready to fall out naturally, because losing a baby tooth too early can cause the other teeth to drift and affect the space available for the adult tooth."

00:08:27: A pulpotomy involves the removal of the coronal pulp — the pulp tissue in the crown portion of the tooth — while leaving the radicular pulp, the pulp in the root canals, intact. Parent-friendly phrase: "The decay has reached the nerve of the tooth. We need to do a small procedure to remove the top part of the nerve and put in a special medicine to keep the roots healthy. Afterwards, we'll put a little silver cap on to protect the tooth."

00:09:10: A pulpectomy, on the other hand, involves complete removal of all pulp tissue from both the crown and the root canals. Parent-friendly phrases: "The infection has reached all the way down into the roots of your child's tooth. We need to clean out the inside of the tooth completely — roots and all — and fill it with a special medicine to stop the infection from spreading. Think of it as a thorough deep-clean for the inside of the tooth." "Imagine the inside of the tooth is like a little tunnel system. The infection has travelled all the way down to the bottom of the tunnels, so we need to clean every single tunnel out, disinfect it, and fill it with a special medicated paste to keep everything healthy."

00:10:22: Here is your overview of the vocabulary of this week’s lesson: to involve – einbeziehen, umfassen; to drift – abweichen, schweifen; to be proud – Stolz sein; thorough – gründlich; poorly – schlecht; tissue – Gewebe; glue – Kleber; buzzing sound – summendes Geräusch; nap – Nickerchen; sticky liquid – klebrige Flüssigkeit.
